Reflexes. Straight and simple. Doesn't matter what's in your hand if you're unable to apply it effectively because you haven't practiced with it. I'm probably far more effective with a keyring of splayed car keys than a knife because that's what I've had to protect myself every time I've been walking to my car alone late at night.
Knives might slow an attacker down if they think you know what you're up to with it, but if you're presented with someone that's just as desperate as you are that might not mean anything.
Reflexes and muscle memory.
